Insurance & Fees

While both Erin and Malia are credentialed or in process with the insurance companies listed below, the specifics of every plan may vary on an individual basis. For this reason, we recommend calling your insurance company prior to your appointment to determine what your costs may be and how many visits may be covered.

We also wish to remind you that if you have a deductible it resets annually and most people will pay the full allowed amount (as set by their insurance plan) for sessions until their deductible is met.

For all patients enrolled with Sequoia Integrative Psychiatry, a credit card is required to be on file which will be billed for co-pays and in the event of no shows/late cancellations.

Paperwork completed outside of a session.

$30 minimum | $100/hour

No Show/Late Cancellation Fee

No show or cancellation within 24 hours of appointment time will incur $100 cancellation fee. If you are more than 15 minutes late to a visit, Sequoia may mark the visit as a no show and bill the cancellation fee.

No Surprises Act

Recent federal legislation requires any patient who is paying out-of-pocket for services or seeing an out-of-network provider must be informed about the No Surprises Act. Additional information can be found here.
